Hello Staff, Parents and Athletes! Welcome back to another school year. Please look over the following schedule of events for activities at CMS:

8/13/12 3-5 PM Registration CMS Cafeteria

8/13/12 5-7 PM Physicals CMS Cafetria $20

Coaches will have detailed information about practice schedules for specific activities at registration. With the change in schedule for the school day most of our activities will be held before school. Current;y only football and cross country practices will be held after school. Guidelines to prepare for the upcoming 2012-2013 school year.

Prior to Practicing with the Team:

1: Physical dated after July 1st, 2012. Physical forms can be picked up at the middle school or downloaded form the "Parent Resources" link on the CMS homepage.

2: Proof of Insurance

3: Warn of Risk, Medication and Regulations Packet.

4: Physical forms are located at the offices of Dr. Hainey (565-6776), Dr. Barkhurst (564-8086, Cortez   Family Practice (565-2007) and Southwst Memorial Primary Care (565-8556).

Prior to receiving a uniform:

1: $45 activity fee cross country, volleyball

2: $55 activity fee football

Keep in mind that we will have limited participation this year. Football will have two teams and volleyball will have three teams. Students will be selected based on their ability to meet the criteria set by the head coach prior to the season. Please call the activities office with any questions.
