Transcript Request

To request a copy of your transcript, please complete the Transcript Request Form below and submit it along with a valid copy of your photo ID to transcripts@cortez.k12.co.us or via fax to 970-565-2161.

Due to federal law, information concerning a former student's record cannot be released until a records release form has been submitted. If a student is under 18 years of age, the form must be signed by a parent/guardian. The request must include the correct mailing ad­dress to which the transcript is to be sent.
